Rwanda: a case of demographic entrapment.

When King published 3 articles about demographic entrapment, he recommended that primary health care stop targeting child mortality, indeed that oral rehydration solutions be withheld, in order to reduce population size and evade the demographic trap of a region endangered by the growth of its own population. Using Rwanda as an example, it is possible to calculate the effect of decreasing child mortality on population growth. From 1970, when there were no programs to reduce child mortality, until 1993 the birth rate remained constant and the child mortality rate slowed so that an extra 0.5/5 children would survive to their fifth birthday. Thus, most of the population growth increase of 0.5% is probably attributable to decreased child mortality. If the high mortality rate had been maintained, the growth rate would have remained at 2.9% and the population doubling time would have been 24.2 years. The actual doubling time between 1970 and 1993 was 22.1 years, so only 2 years would have been gained by increased child mortality. The problem in Rwanda was not decreased child mortality, therefore, but the constant birth rate. Rwanda has a very Catholic population which embraces the Roman Catholic Church's anti-family planning policy. It is very hard for illiterate poor to refute the teachings of the church. The collapse of Rwanda caused by its overpopulation presents an example to the world of the devastating consequences of denying effective family planning to the poor.
